Your First Visit

Easily Prepare for your first visit to Spectrum Physical Therapy! Below is everything you'll need to ensure that your check-in goes smoothly.

Paperwork is available below, and may be filled out prior to your appointment. In addition to completing the paperwork, please remember to bring with you your prescription, insurance card(s) and ID. You can expect your initial appointment to last 1.5-2 hours. Remember to wear appropriate clothing to work out in, i.e., sneakers, gym shorts, t-shirt, etc.  Please arrive to the office 15 minutes prior to your scheduled appointment time to fill out any necessary paperwork.

At your initial appointment, your therapist will go over your medical history, current injury and you’re your treatment goals. They will perform a number of functional assessments in order to determine your current capabilities and performance levels. Once your therapist has a thorough understanding of your condition/injury, treatment will include an exercise program personalized to your injury, manual therapy, and education you on your pain/condition. You’ll also receive a home exercise program to empower you to self-manage and treat your pain. Each aspect of your care will be uniquely tailored to your ability, goals, and interests.

Spectrum Physical Therapy accepts most major insurance companies. As a courtesy to our patients, our administration staff will verify your insurance benefits prior to the first appointment. Please keep in mind that verification of benefits through your insurance company is not a guarantee of benefits, and may change as claims are processed. Our team works diligently to ensure that our patients are receiving the most from their insurance.
