Easily prepare for your first visit to Spectrum Physical Therapy! Below if everything that you will need to ensure that your check-in goess smoothly.
Prepare for your first visit
Paperwork is available below, and may be filled out prior to your appointment. In addition to completing the paperwork, please remember to bring with you your prescription, insurance card(s) and ID. You can expect your initial appointment to last approximately 1.5 hours.
Remember to wear appropriate clothing to work out in, i.e., sneakers, gym shorts, t-shirt, etc. Please arrive to the office 15 minutes prior to your scheduled appointment time to fill out any necessary paperwork.
At your initial appointment, your therapist will go over your medical history, current injury and you’re your treatment goals. They will perform a number of functional assessments in order to determine your current capabilities and performance levels. Once your therapist has a thorough understanding of your condition/injury, treatment will include an exercise program personalized to your injury, manual therapy, and education you on your pain/condition. You’ll also receive a home exercise program to empower you to self-manage and treat your pain. Each aspect of your care will be uniquely tailored to your ability, goals, and interests.

If you are visiting the office for a lower body issue involving the legs, knees, ankle, or foot, please fill out the following forms and bring them into the office with your new patient paperwork.
Knee Injury Scale (only if you are coming in for a knee injury)
If you are visiting the office for an upper body issue involving the arms, hands, shoulders, or neck, please fill out the following forms and bring them into the office with your new patient paperwork.
Neck Scale (If pain from shoulders radiates to neck)
If you are visiting the office for a lower back issue, please fill out the following forms and bring them into the office with your new patient paperwork.
If you are visiting the office for an upper back issue involving the shoulders, neck, and mid-upper spine, please fill out the following forms and bring them into the office with your new patient paperwork.
Arm Should Hand Scale (if neck pain is radiating into shoulders and arms)
If you are visiting the office for an issue involving the hips, please fill out the following forms and bring them into the office with your new patient paperwork.
All medicare patients must fill out the Falls Assessment. Please fill out scales that correspond to your injury as well as the following form and bring in with your paperwork.
